This Agreement is between Jack & Associates LLC ("Consultant") and the undersigned Client ("Client").

1. SERVICES PROVIDED
Consultant will:
(a) Register Client’s business entity as an "Organizer."
(b) Obtain an EIN from the IRS.
(c) Provide a basic Operating Agreement for general business use.

2. CLIENT RESPONSIBILITIES
Client acknowledges:
(a) Consultant acts only as an Organizer and is not responsible for business management.
(b) The Operating Agreement created is for general business use; legal review by an attorney is recommended for clients seeking a more robust operating agreement unique to their specific business.
(c) Consultant does not provide legal or financial advice.

3. LIMITATION OF LIABILITY
Consultant is not liable for any legal, financial, or operational issues arising from Client’s business activities.

4. ACCEPTANCE OF TERMS
By submitting this form, Client agrees to these terms.
